AGREEMENT:
I understand this form must be completed and received by Media City Church prior to the beginning of the Three-Month Generosity Challenge Period.
I understand my household qualifies for participation because we have not been giving this increased amount for the last three months.
I understand I cannot seek a refund prior to the end of the Three-Month Generosity Challenge Period, and that I cannot seek a refund for any contributions made prior to the beginning of the Three-Month Generosity Challenge Period.
I understand any request for refund must be received by Media City Church within 30 days of the end of the Three-Month Generosity Challenge Period.
I understand that if paid physically at Media City Church's campus or by mail, my gift/s must be paid by check, or by completed offering envelope, so that my gift/s can be properly credited.
I understand that if paid physically at Media City Church's campus or by mail, my gift/s must be designated with a memo related to “Three Month Generosity Challenge”.
I understand that if paying online, I must enter accurate personal information prior to giving my gift/s so that my generosity can be properly credited.
I understand that if paying online, my gift/s must be designated under the fund category “Three Month Generosity Challenge”.
I would like to grow in the habit of generosity by accepting the Three-Month Generosity Challenge. I agree that for the three-month period, my household will contribute to God, through Media City Church, a decided gift amount or percentage in conjunction with personal obedience to Jesus. At the end of the three-month period, if there is still any need for the gifts given, I will be entitled to request a refund of the full amount of contributions made during that three-month period.
